Chronic Conditions Education Programs
Welcome to our Chronic Conditions Education web site! If you have an on-going medical condition or illness such as diabetes, heart disease, heart failure or asthma, our chronic conditions education programs can help you make healthy decisions.
Whether you are new to Kaiser or have just been diagnosed with diabetes, heart disease, heart failure or asthma, our Chronic Condition Education classes offer a balance between information and practical life skills; all of which are integrated into part of your overall health care experience. All of our Chronic Conditions education classes include scientifically proven medical, nutrition, and health information, as well as interactive activities and self-management tools.
Our Instructors
Our instructors have advanced degrees in nutrition and/or a health related field. In addition, many of our instructors are Registered Dietitians and may be Certified Diabetes Educators. All of them have had extensive training in group education and facilitation and working with families and individuals. Our Chronic Condition Education Coordinator oversees all of our classesand periodically updates the class content to assure up-to-date health and nutrition information.
Programs and Services
Choose from a variety of single or multi-session classes such as our "Cholesterol & Your Heart Classes", "Salud Para Su Corazon", "Living Well With Diabetes", "La Diabetes Y Su Salud", "Understanding Your Asthma" and more.
As a Kaiser Permanente member, you may enroll in these classes through the health education department, your clinical health educator or medical provider. If you need additional help with lifestyle goals we offer individual appointments with our clinical health educators. Or if you need specialized nutrition care your physician can refer you to an individual consultation with one our registered dietitians.
Our health education center also offers a wide variety of resources on women's health. Please stop by to pick up free pamphlets, use our computer to search for scientific articles, or to borrow or purchase books, audios, and videos.
